POI – Place of Injury

DOI – Date of Injury

TOI- Time of Injury

B/B – Brought by (the person who brought the victim)

AMPLE history

A- Allergy

M- Medication received for trauma anywhere and what

P- Past history and pregnancy

L – Last meal – how many hours before he took meal ( help us in securing airways and predicting chances of aspiration)

E – Environment leading to trauma ( Road, high speed, alcohol or weather etc.)
